New Client Forms
For your convenience, new client forms are available below for download so that you can fill fill them out before coming to your first appointment. Please select the adult/child intake form as appropriate for you or your child. The Hippa and Philosophical Agreements need to be read and signed as well by every new client. Please plan 45 minutes - 1 hour for your first visit.
